A wide range of special services helps to ease the pressing concerns of patients and their families. Our aim is to anticipate our patients’ needs as they recover. With fewer worries about details like laundry, cooking and cleaning, patients can concentrate on rehabilitation and healing.
As patients near a homecoming date, our Social Services Coordinator can help arrange for independent living support through community agencies, such as Meals on Wheels or the Council on Aging, or by making referrals to companion services and home health providers. Once a patient leaves the Senior Rehabilitation Center, he or she returns to the care of a primary physician. Appropriate medical records regarding therapeutic treatment are transferred upon discharge. Occasionally, patients may not progress enough to return to independent living. In that case, patients and their families may choose to remain at Limestone Health Facility for long-term restorative nursing care.
